This form is used to report sources of income, real estate transactions, bank accounts,
stocks, debts owed, business holdings and business customers.
Due Date: Within 2 weeks of becoming a candidate or being newly
appointed. Incumbent elected and appointed officials report annually between January
1 - April 15. Any local elected official whose term of office expires immediately
after December 31 shall file a statement of financial affairs for the calendar year
which ended on that date.

of food, travel, and education payments from anyone outside of their agency.

A shorter version of the Personal Financial Affairs Statement (F-1) that may be
used by annual filers who have filed a full F-1 within the last three years if the
information reported on the full F-1 is still accurate or needs only minor changes.
Due Date: Elected officials must file between January 1 and April
15 during the year they hold office. Any local elected official whose term of office
expires immediately after December 31 shall file a statement of financial affairs
for the calendar year which ended on that date. Appointees to elective office must
file within two weeks of being appointed.
C1 Candidate Registration - (MS Word Template),

name, time and place for public inspection of records.
Due Date: Within 2 weeks of becoming a candidate. For PDC purposes,
someone may become a candidate well before a formal declaration is filed with elections
officials.

This form is used to report names of contributors, amounts, addresses, occupation,
employer.
Due Date: All monetary contributions must be deposited within 5
business days of receipt; fill out a C-3 for each deposit. Prior to June 1, C-3
is due monthly; after June 1, file C-3 reports weekly on Monday.

This form is used to report total contributions and expenditures for defined period

Due Date: Summary of Receipts and Expenditures due with initial
C-1 report if contributions were received or expenditures made prior to registration
and on the 10th of each month covering the proceeding months activity through June
10th of the election year.

Identifies recipients of and contributors giving $1000 or more immediately before
primary and general elections
Due Date: Contributors report within 24 hours of or the first working
day after giving $1000 or more during 7 days before primary election and 21 days
before general election. Recipients report within 48 hours of or the first working
day after receiving $1000 or more during 7 days before primary election and 21 days
before general election.
Special Report E Earmarked Contribution - (PDF)
This report highlights receipt of earmarked contributions given by any contributor
with the understanding, intent or instruction that is to benefit another candidate
or committee.
Due Date: The report is due within two working days of receiving
the earmarked contribution. Also send a copy to the candidate or committee benefited
within two days.
