These forms will help your committee meet its reporting requirements.

Did you spend over $10,000?

Committees that expended $10,000 or more the preceding year or
expect to expend $10,000 in the current year are required to file campaign finance
reports (C3 and C4) electronically. PDC staff provides free electronic
filing software and training to all committees. |

C1pc Political Committee Registration - (MS Word Template), (PDF)
Political committee registration. Includes the purpose of the committee, reporting
options, treasurer's name, relationships with other political committees or candidates,
identifies committee officers, and name of financial institution.
Due Date: Within 2 weeks of becoming a candidate. For PDC purposes,
someone may become a candidate well before a formal declaration is filed with elections
officials.
C3 Cash Receipts - (

This form is used to report names of contributors, amounts, addresses, occupation,
employer.
Due Date: All monetary contributions must be deposited within
5 business days of receipt; fill out a C-3 for each deposit. Prior to July 1, C-3
is due monthly; after July 1, file C-3 reports weekly on Monday.
Schedule L Loans - (MS Word Template), (PDF)
Details regarding loan payments and loans forgiven and outstanding.
Due Date: File with C3 when loan is deposited.
Attachment Au Auction Report - (MS Word Template), (PDF)
Identifies contributors donating and purchasing auction items. Includes name,
address, of the contributor and the market value and purchase price of the auctioned
item or service
Due Date: File with C4
C4 Summary, Full Report - (MS Word Template), (PDF)
This form is used to report total contributions and expenditures for defined period
and overall campaign.
Due Date: Summary of Receipts and Expenditures due with initial
C-1pc report if contributions were received or expenditures made prior to registration
and on the 10th of each month covering the proceeding months activity through July
10th of election year.

If a continuing committee is not supporting or opposing candidates or ballot measures
this year, it only files C-3 and C-4 reports on the 10th of the month if contributions
or expenditures exceeded $200 since the last C-4 report was filed. No 21- or 7-
day or post-election reports are required. Candidates participating
in special elections file C-4 reports 21 and 7 days before and on the 10th of the
first month following the election. |

OTHER FORMS
Last Minute Contributions of $1000 or More - (MS Word Template)
Identifies recipients of and contributors giving $1000 or more immediately before
primary and general elections
Due Date: Contributors report within 24 hours of or the first working
day after giving $1000 or more during 7 days before primary election and 21 days
before general election. Recipients report within 48 hours of or the first working
day after receiving $1000 or more during 7 days before primary election and 21 days
before general election.
Special Report E Earmarked Contribution - (PDF)
This report highlights receipt of earmarked contributions given by any contributor
with the understanding, intent or instruction that is to benefit another candidate
or committee.
Due Date: The report is due within two working days of receiving
the earmarked contribution. Also send a copy to the candidate or committee benefited
within two days.
