L1 Lobbyist Registration - (MS Word Template), (PDF)
Identifies lobbyist and employer. Indicates employment status and financial arrangements.
Shows general subject areas of expected lobbying activities.
Due Date: Within 30
days of employment as lobbyist or before lobbying, whichever occurs first.
L1 Photo ID Form

As part of the registration process, each lobbyist must provide a brief biographical
sketch and a passport size (2" x 2"), glossy photo. The photo must have been taken
within the last 12 months. Be sure to include the full name of all employers for
whom you will be lobbying on the ID Page as well as all the other information.
Due Date: Within 30 days of employment as lobbyist or before lobbying, whichever
occurs first.
L2 Lobbyists Monthly Expenses

Shows lobbyist's expenses and compensation. Includes employer's expenditures on
behalf of lobbyist to assist in the lobbying effort. Due Date: Monthly by the 15th. Covers preceding calendar month.
L-2 Memo - (MS

Reports and provides notice to recipients of contributions and other items of value.
Used in lieu of reporting this information on L2 and giving recipients copy of L2 report. Due Date: With L2 (Optional)
L3 Employer Report

Lobbyists who hired or contracted with other lobbyists must file this report. Anyone who employed a lobbyist during calendar
year 2005 should file the L3 report. Includes information
not required of the lobbyist(s). Due Date: (Annualy) By the last day of February covering compensation
and expenditures paid or accrued in the previous calendar year.
L-3c Employer's Contribution Report

Identifies recipients and amounts of contributions exceeding $100 made by employers
of lobbyists. Due Date: within 15 days of the end of any month during which reportable contributions
were made.
L7 Employers of State Legislature,
State Officers or State

Lobbyists who hired or contracted with other lobbyists must file this report.
Indicates when a lobbyist employs a member of the legislature,
an employee of the legislature, a member of a state board or commission, or a full
time state employee, if that employee remains partially employed by the state. Due Date: Within 15 days after commencement of employment.
LMC Last Minute Contributions - (MS Word Template)
During the last three weeks before the general election, report within 24 hours of making a contribution or an aggregate of contributions to a single entity that is $1,000 or more.
